Each Parish/Team must complete the League_Checklist.doc  This form can be emailed to ejrobinette@gmail.com.  Form must be received prior to the first game of the season.

Head Injury Form

The head injury form must be completed and signed by the coach, qualified health care professional, and parent or guardian before any player that recievs a potential head injury can return to that, or any future, game.Head_Injury_Form_1.pdf Head_Injury_Form_2.pdf
